Insurance Policy

We participate in most insurance plans and will see patients out-of-network if eligible. Please call the office for an up-to-date list of participating insurance plans. All patients are responsible for providing accurate insurance information at the time of visit, including a copy of the most recent insurance card.

Health benefits cannot be confirmed until your claim is processed by your insurance carrier and is based upon medical necessity. The information received from the insurance company prior to your treatment is not a guarantee of payment. Please call your insurnace carrier if you have any specific questions about coverage.

Payment Policy

Payment for all co-pays, deductibles and self-pay balances are due at the time of visit. Our office accepts credit card, cash or personal check with driver's license.

Cancellation Policy

We understand that you may need to cancel an appointment occasionally. Please provide our office a 24-hour notice in the event that you need to reschedule your appointment. This will allow us the opportunity to provide care to another patient. A message can always be left with the answering service to avoid a cancellation fee being charged. A “No-Show”, “No-Call” or missed appointment, without proper 24-hour notification, may be assessed a $25 fee. This fee is not billable to your insurance. If you are 15 or more minutes late for your appointment, the appointment may be cancelled and rescheduled. As a courtesy, we make reminder calls, for appointments, one to two days in advance. Please note, if a reminder call or message is not received, the cancellation policy remains in effect.
